CareCoord: a multi-agent care coordinator

What I built as a finalist at Anthropic's Built with Opus 4.7 hackathon: parallel specialist agents that read a family's medical papers and connect what no single doctor sees.

Top 2.4% of 20,000 global applicants. The organisers compared the bar to YC and the most selective universities on earth.

I was honoured to be one of the builders selected for the Built with Opus 4.7 hackathon by Anthropic and Cerebral Valley, and I spent the week building and shipping CareCoord.

It's a multi-agent care coordinator for the 1 in 6 adults worldwide caring for an ageing or chronically ill parent.

Photograph any medical paper, in any language. Multiple Claude Opus 4.7 specialist agents analyse it in parallel. The orchestrator finds the cross-domain connections no single specialist sees.

A diuretic from the cardiologist. An NSAID from the GP. A creatinine number quietly climbing across three lab slips nobody connected.

CareCoord caught the drug pair driving the kidney decline and produced the clinical handoff in Spanish, in 90 seconds, with every source cited.

From that single analysis, the engine renders five distinct documents (caregiver brief, SBAR handoff, insurance appeal, attorney evidence pack, bedside note), re-rendered instantly when you toggle the audience.

Under the hood

  • Five domain specialists fan out in parallel; one worker failing never aborts the run.
  • A deterministic verification layer (readability floor, jargon detection, schema validation) checks what the agents produce, because self-grading agents approve their own mistakes.
  • Prompt caching on the shared prefix, a hard cost ceiling, an iteration cap and a circuit breaker. A full analysis costs about four cents.
  • The orchestrator depends on an interface, not the SDK, so 417 tests run with zero live API calls.

Why it matters

Building alongside the world's top Claude Code teams was a privilege. But the problem doesn't care about hackathons. About a billion people are 60 or older, and fewer than half live in countries with a national EHR API a consumer app can call. CareCoord is for them.
